Creating a new questionnaire

You can create one or more questionnaires using the question library and/or your own questions. It is also possible to reuse old questionnaires to create a new one. 


Questionnaire tool overview

Follow these first steps to create a new questionnaire:

  1. Log in to www.praxar.com, and go to the Tools tab in your INSTRUCTOR ZONE.
  2. Select the ''Questionnaire'' tool, and click on ''Create a questionnaire''. 
  3. In the pop-up window, enter a name for the questionnaire, and click on “Create”.

4. You are now in the Questionnaire Editor.

Here is an overview of this tool:



1. Import: Use this option to import individual questions from the question library or past questionnaires. You can also import an entire questionnaire if needed.

2. Add a question: Click here to create your own custom question from scratch.

3. Shuffle questions: Use this feature to randomize the order of questions when the questionnaire is presented to participants.

4. Shuffle answers: This feature randomizes the order of answer choices within each question when displayed to participants.

5. Publish: This is the last step of the creation process of your questionnaire. Once you're done with your questionnaire, you must publish it before planning an evaluation period with this questionnaire.

6. Display the versions: Use this feature to view all questionnaire versions with their date of creation.

Using the question library


By clicking on the ''Import'' button, a new window will be displayed:


  1. Choose to import questions from:
    1. Normal library  
    2. Old questionnaires.
  2. Select a questionnaire in the left menu.
  3. In the questionnaire, select the questions of your choice.
  4. Click on ''Confirm'' to import your questions.

Using your own questions


Click on the ''Add a question'' button to show the question builder in the left menu:



  1. Write a question, and then your answer choices.
  2. Check the box of the correct response.
  3. Enter the score for this question.
  4. Click on ''Close Editor'' to add this question to your questionnaire.

Next steps

Review your questions, and once you're satisfied with your questionnaire, simply click on the ''Publish'' button at the top of your screen:


1. In the pop-up window, enter the name of this version, and click on ''Publish''. 
Warning
Once published, this version of the questionnaire can not be modified. However, you can edit and publish a new version of the same questionnaire.

2. After publishing a questionnaire, it is possible to use this questionnaire to schedule an evaluation. For more information about it, see Scheduling evaluation periods for questionnaires.

3. Once students have completed their questionnaire, you can view their results using the Reports tool or the Grading report.
